Program Spotlight: Nursing Home Transition
The goal of CILCP’s Nursing Home Transition (NHT) Services is to move individuals with disabilities from nursing homes into independent, community-based living. Last week, this program added another complete transition to their list of success stories—thanks to Kaori Kelly, Nursing Home Specialist (Learn more about Kaori here).
The Miller family had been waiting a long time for Walter to return home. CILCP was able to provide the comprehensive array of services needed to make this happen. Kelly coordinated Personal Assistance Services (PAS), Home Modifications & Access Services, and Living Well Transport Services.
Lynn Stewart-Krieger, Director of Home Modifications explained, “Mr. Miller lives in the city and needed a safe way to access his home. Our agency met with Walter, his family and the landlord to determine the best option to get in and out of his home. We decided that the Vertical Platform Lift (VPL) will allow him to do that safely.”
After Walter’s Home Modification was complete, the Living Well Transport Services van picked him up from his nursing home in Lebanon and brought him to his now accessible home in Harrisburg.
After seven months of coordination, advocacy and determination, the Miller family is reunited.
